CMS Lists 5 Louisiana Nursing Homes as "Poor Performers"

The Centers for Medicare & Medicaid Services (CMS) has published a previously undisclosed list of nursing home facilities that have failed to meet the crucial health and safety requirements mandated by the federal government. The current list includes over 400 underperforming facilities that have been frequently cited for safety violations and quality of care deficiencies.

According to CMS, “Most nursing homes have some deficiencies, with the average of being 6-7 deficiencies per survey” and when “deficiencies are identified, we require that the problems be corrected. If serious problems are not corrected, we may terminate the nursing home’s participation in Medicare and Medicaid.”

Every facility identified on this list is assigned a “Special Focus Status”: Special Focus Facility (SFF) or SFF Candidate. SFF nursing homes are eligible for participation in the Special Focus Facility Initiative, a program that encourages quality of care improvements through increased site inspections (about twice per year) and civil monetary penalties.

The 2019 list includes 5 Louisiana nursing homes that are eligible for the SFF program:

  • Belle Maison in Hammond (SFF)
    • Failure to provide patients with a safe living environment
    • Failure to prevent resident-to-resident abuse and mistreatment
    • Failure to treat wounds that developed into ulcers/bed sores
  • Lake Charles Care Center in Lake Charles (SFF Candidate)
    • Failure to provide patients with a safe living environment
    • Failure to meet proper hygiene standards
    • Failure to manage an infestation (insects)
  • South Lafourche Nursing and Rehab in Cut Off (SFF Candidate)
    • Failure to provide patients with a safe living environment
    • Failure to meet proper hygiene standards
    • Failure to manage an infestation (insects)
  • St. Helena Parish Nursing Home in Greensburg (SFF Candidate)
    • Failure to administer life-saving procedures, including CPR
    • Failure to follow a resident’s care plan
    • Failure to properly train staff in their duties
  • Tioga Community Care Center in Pineville (SFF Candidate)
    • Failure to prevent patient malnourishment and dehydration
    • Failure to provide patients with a safe living environment

Unfortunately, only 88 of the 400 facilities will be included in the SFF program due to federal budget constraints. For this reason, it’s vital that families thoroughly investigate a nursing home or assisted living facility before starting the admittance process.

Belle Maison

The Louisiana Department of Health (LDH) first identified Belle Maison as a Special Focus Facility in 2018. Emily Woodruff, a writer for The New Orleans Advocate, wrote an article about the negligent Louisiana facility, explaining that “inspection reports conducted by the Louisiana Department of Health noted failures to protect residents from abuse and failures to provide appropriate wound checks to keep ulcers from forming.” Woodruff also analyzed a 2017 case, where a mute paraplegic resident was nearly smothered to death by a roommate on two separate occasions. Facility staffers didn’t separate the patients and never notified their families about the attacks.
